Residential Shelter Bilingual Case Manager, East Valley
A New Leaf is growing! We are in process of opening a newly licensed and accredited 24-hour youth shelter for children (ages 12-17) and Foster Care Program, that will offer specialized behavioral care and case management for the purpose of family reunification. Meals, education and a temporary home are provided along with activities to keep the children’s spirits up and deliver round-the-clock care.
A New Leaf's dedicated staff provide a safe and stable environment, critical for optimal growth and development of each child. Supporting objectives include; improved physical health, education/vocation skill development and timely family reunification and/or release planning.
We are searching for multiple full time Spanish/English bilingual Case Manager V to join our new youth services and be a part of the already amazing and dedicated team.

Rate of Pay: $19.00/hour
Full-Time, 8:30am - 5pm, Sunday - Thursday
Full-Time, 8:30am - 5pm, Tuesday - Saturday
Major areas of responsibility:
Responsible to the supervisor for administrative and clinical issues regarding admission, discharge and utilization of management issues.
Maintain case management files, to assure they meet The Office of Refugee Resettlement (ORR) and agency-reporting standards; including UAC Portal documentation and agency paperwork in a timely, correct, and thorough manner.
Develop and maintain collaborative relationships with clients, their family members and significant support systems. Meet frequently with clients in person and family members via telephone. Provide up-to-date information on case status and new developments regarding progress and discharge planning.
Co-develop Safety/Care Plans for clients. Coordinate and assist with completion of all necessary documentation as designated by ORR and agency standards.
Assist clients and families in obtaining community services, securing appointments with outside services and agencies as needed.
Assess and evaluate strengths, weaknesses and safety of the client’s future placement. Assist in developing plans to strengthen placement as needed.
Facilitate and coordinate discharge planning transition and follow-up according to plan.
Maintain case management files. Updating and completing staffing notes for multi-disciplinary clinical and case management team reviews and ensuring that the treatment plan is carried out.
Document (verbal/written) and submit significant and routine paperwork such as progress notes, statistical reports, daily activities, upcoming events, special concerns, varied incident reports.
Bachelor’s Degree in Behavioral Health plus two (2) years recent behavioral health experience.
Spanish/English bilingual required and have the ability to communicate effectively, in written and verbally in Spanish and in English as well as interpret and translate.
Possess a valid driver’s license with reliable transportation, a clean 39-month motor vehicle record, current auto liability insurance and a Class One Fingerprint Clearance Card.
Must have proficiency with personal computers and applicable software programs. Must have the skills necessary to engage and work with others from diverse ethnic and cultural groups, coupled with maintaining professionalism at all times. Must demonstrate and provide strong communications skills both verbally and in writing. Must be committed to teamwork and have the ability to establish and maintain effective working relationships.
